Colonial CFJT Client Agreement
This is a ten week-once a week-anger management program cosponsored by Colonial Community Corrections and the Center for Therapeutic Justice designed to assist you in understanding, addressing, resolving, and communicating your anger, issues, and needs in an appropriate way. While in the program, you will be expected to follow these program and group rules:
- Come to the group sessions on time. If you are over ten minutes late, you may stay, but you will not get credit for the group. Use Personal Attendance Form (handout) to keep track.
- Attend 10 full group sessions (1.5 hrs. x 10 = 15 hrs.). Absences can only be excused by your caseworker: Call if you expect to be late or absent so your intention to continue is known.
- More than one absence necessitates your starting over in the next 10-week group, and there is no refund. A $15.00 fee for the make-up session is required.
- You will be asked to pay a $40.00 commitment fee to your caseworker before starting group or on the first night. This will be applied to your total fee due ($185.00).
- Fees in the form of money orders only made out to JCC/CCC will be collected before group. JCC/CCC will provide receipts for payment.
- You will be required to use a book to complete your Self-Work assignments (written). The book will be available for $12.00. There is up to a $10.00 reimbursement if the book is returned.
- You will be asked to sign a release of information form so that CFTJ and CCC can communicate about your participation in this program.
- No one will complete this group or receive a completion report until ten sessions are attended, all Self Work (written) assignments are finished, and group fee is paid in full.
- No excessive profanity, threats, disruptive behavior or derogatory, racial, ethnic comments will be tolerated during group session or on site. Cell phones and pagers must be turned off.
- The identity of all participants will be kept confidential, and personal information discussed in group must, also, be kept confidential.
- Group members will participate in each group session and support the group by getting other group members involved in the group process by sharing their experience.
- Anyone under the influence of mood altering chemicals (including alcohol) will not be allowed to attend the group session and this will be reported to the agency.
- All group members will be subject to random breathalyzer tests during group sessions and results will be reported to the agency.
- Group members will be asked to pursue additional services if substance abuse, domestic violence, or mental health problems are identified and further assessment is recommended.
You will be asked to take a good, hard, long, and honest look at yourself during this program and asked to evaluate the group and your own progress at the last session. I, ______________________________________________ have read or have had read to me the rules for my participation in the anger management group. I understand these conditions, and I agree to abide by them at all times. If I violate these conditions, my caseworker will be notified, and I understand that I may be asked to leave the group. This might return my case to Court and in addition there might be an imposition of immediate sanctions by my caseworker, if deemed necessary.
