Preparing for Benefits Orientation

Full-time Regular Employees

To help prepare you for your Benefits Orientation, please review the following information prior to your benefits meeting to prepare you to make the following decisions:

If you want to…	You need to…
Enroll in health insurance	Decide which health plan you want and choose a Primary Care Physician from the Optima Provider Directory or the Anthem Provider Directory. When searching Optima provider directory use the HMO(except OFC/FAMIS) option. When searching the Anthem provider directory use either Anthem KeyCare (PPO) or Anthem Healthkeepers (Peninsula Health Care, Inc.-HMO) options.
Enroll in dental insurance	Decide which dental plan you want and choose a dentist.
Waive health insurance	Complete the Health Insurance Waiver Form.
Waive health insurance, but enroll in dental	Complete the Health Insurance Waiver Form and a Dental Enrollment Form.
Enroll in Flexible Spending Accounts	Complete an enrollment form, Flexible Spending Account Direct Deposit Form, and Flexible Spending Debit Card Enrollment Card.
Enroll in Deferred Compensation	Complete the enrollments forms.
Enroll in Optional Life Insurance	Complete the Optional Life Insurance Enrollment Form.
