This form must be completed in its entirety and received in the JCSA business office located at 101-E mounts Bay Road, Williamsburg, VA 23185 no later then the 10^th of the month indicated on the submeter forms that were mailed to you after we received your registration form. The submeter form indicates what date and months to submit the information to our office. Should a reading be received late the credit will be delayed until the next billing cycle. Submeter deductions given for any one billing cycle can not exceed the total consumption for that billing period. No back credit will be given.	Submeter Reading
